I think your information is dated.  Wal-Mart has significantly improved the healthcare they provide employees.  Keep in mind of the 1.6 million people they employee, not all are full time employees.  I am not aware of any company that provides healthcare to part time employees, but Wal-Mart does have a very cost effective package they can purchase.  Some have coverage from their spouse and some choose not to have coverage.

I know it is common for employers to provide health coverage with a co-pay to their employees .... but I for one have never felt it is an "employers" responsibility to provide coverage.  Why?   Do they provide home owners or renters insurance?  Do they provide auto insurance so you can drive to work? 

It should be the individuals responsibility to care about themselves and their family to provide coverage ... and yes I feel rates should be based on the lifestayle of the person.  You pay more if you smoke or over weight.  But coverage can not be denied and if below a certain income level they can have government subsidized assistance for health care.  But heathcare itself should be run by and stay in the private sector.
