I think the reality in this hyperpartisan world is that many on the right were glad he didn't shake her hand and many on the left were glad she tore up his speech for all to see. I would point out that he didn't shake the Vice President's hand either and if you watch it in real time it's not even clear to me that he saw her extend her hand before he turned. But him not shaking the Speaker's or VP's hands before his speech will be forgotten but her tearing up his speech will be the GIF gift of 2020. If he intentionally ignored her handshake out of spite I suspect he would have shaken the VP's hand. I simply think he didn't see her extend her hand and forgot but only he knows for sure.
