A tooth paste company was having a problem with an occasional empty carton. They decided to hire an outside engineering / manufacturing firm to solve the problem. Eight million later they had a precision scale to weigh each carton as it progressed down the line. When an empty was found the line stopped, a bell sounded and a worket walked over to remove the empty carton and push a button to restart the line.
The soon the empty carton problem went away. Money well spent, right? Upon further checking they found the machine had found zero rejects when there should have been a number. Investigating further up the line they found a fan blowing across the line. Empties were blown off. The operator explained that they got tired of walking over to remove the empty and restart the line.
